We Are Relocating
This note is to inform you that we–Dr. Mary Ashmead and Amy McDonald, PA-C– are relocating our practice from Texas ENT to start our new ENT office nearby in Southlake. I am confident that this move will enable us to continue providing excellent care and service to our patients. Our new office will be located in east Southlake at 300 S Nolen Drive, Suite 100. The new office is scheduled to open July 17, 2023. Our last day at Texas ENT is July 11th. We are excited about this transition and the opportunity to continue serving you and your healthcare needs in a new setting. We understand that continuity of care is of utmost importance and want to provide options to ensure seamless medical attention going forward.
You can choose from the following alternatives:
1. Transition to the New Practice: You are welcome to transfer your care to our new practice. We can continue our doctor-patient relationship with minimal interruption. You will need to complete the records request form for your records to transfer.
2. Remain with the Texas ENT Specialists: If you prefer to stay with the current practice, we completely understand. The administrative team will be happy to assist you in assigning a new doctor. Your records will remain at Texas ENT and will be available to your new Texas ENT physician.
Regardless of the choice you make, please be assured that your medical records will be handled with the utmost confidentiality and in compliance with all privacy laws and regulations. If you decide to continue your care at the new practice, please let us know as soon as possible.
